Fill out the form. Attach all 5 UPCs, your receipt(make sure to make a copy of your receipt, UPCs and form for your own records) with your purchases highlighted in some way, as well as a  self addressed stamped envelope. They will send you a $10 gift card in 4-6 weeks.
